ABSTRACT

Day surgery can effectively improve the utilization of medical resources. In October 2015, an obstetrics and gynecology hospital established a gynecological day surgery center to centrally manage gynecological day surgery and continuously optimize the management process. In July 2019, the hospital established an intelligent information platform for gynecological daytime surgery managemen. Based on this platform, the " evaluation-appointment-hospitalization-follow-up-chronic disease management" information management process was implemented, the " pre hospital-in hospital-post hospital" full process medical quality and safety monitoring was carried out to form a centralized closed-loop management mode for gynecological day surgery led by gynecologists. Under this management mode, the number of gynecological day surgeries in the hospital has increased from 2 866 cases in 2019 to 4 065 cases in 2021, providing convenient medical services and personalized chronic disease management services, and ensuring the quality and safety of gynecological day medical care, for reference for promoting the high-quality development of day surgeries in specialized hospitals.

ABSTRACT

In order to achieve the goal of integration, homogeneity, a net and a center of medical groups in Hainan province, Hainan province built an integrated management information platform of urban medical alliances and county medical communities based on projects such as the provincial three medical linkage information platform in February 2022. The overall architecture of the medical alliance and medical community platform was divided into system integration layer, data center layer, service layer and application layer. The service layer included four major systems: unified operation management platform, business center, performance evaluation management platform and comprehensive supervision subsystem, which was the core hub connecting the data center layer and application layer, and also was the unified operation and closed-loop management platform of the medical alliance and medical community. As of November 2022, the platform had covered 31 medical alliances or medical communities in 19 cities and counties of Hainan province, achieving information exchange, personnel interaction, resource sharing, business collaboration, and data integration among member units, narrowing the gap between urban and rural medical and health services, which had preliminarily achieved homogeneous management of the medical alliances and medical communities throughout the province, for a reference for the integrated management of medical alliances and medical communities within the regional scope of China.

ABSTRACT

Objective:To explore the development, application and practical experience of investigator-initiated integrated clinical research information platform.Methods:The process of developing and constructing an integrated clinical research platform in a tertiary hospital in Beijing was introduced, the functions and advantages of the platform were described and displayed, and the main problems and risk points in the development and construction process were analyzed.Results:The integrated clinical research platform meets the management requirements of clinical research initiated by investigators, and the standardized management of the whole life cycle of the project can be realized through the platform, and the key issues of data security, information capture, sharing and interoperability need to be further explored in terms of platform docking.Conclusions:The integrated clinical research platform effectively improves the standardization, management quality and efficiency of investigator-initiated clinical research.

ABSTRACT

Objective:To analyze the application effect of the information platform based on the Internet of Things (IoT) in the screening and management of patients with chronic obstructive pulmonary disease (COPD).Methods:In this cross-sectional study, 151 062 residents who participated in the screening in the districts covered by 33 general hospitals and 289 primary medical institutions within the framework of Henan Provincial Center for COPD Prevention and Treatment from November 2021 to October 2022 were included as the subjects. Spirometer based on the IoT was used to conduct pulmonary function tests for subjects who scored 16 points or more with COPD Screening Questionnaire (COPD-SQ), and the subjects were evaluated and managed through the structured electronic data in the information platform. The distribution characteristics and follow-up of the subjects and COPD patients were described and the application effect of this strategy in patients screening and management was analyzed.Results:A total of 151 062 residents with complete basic information in the information platform completed the questionnaire. High-risk population of COPD accounted for 26.5% (40 042/151 062) of the population who received the questionnaire screening, and more than 50% had respiratory symptoms, such as chronic cough (59.4%), or shortness of breath (77.6%). The proportion of high-risk population increased with age, especially after 50 years old. Compared with non-smokers, the proportion of high-risk group was significantly higher (77.1% vs 16.4%) in the group with smoking index over 600. Biofuel exposure (61.3% vs 22.1%) and family history of respiratory diseases (64.2% vs 22.6%) were associated with an increased proportion of high-risk groups, with statistically significant differences ( P<0.001). 5 268 patients were diagnosed with COPD by pulmonary function tests, and the prevalence of COPD in high-risk groups was 27.8% (5 268/18 965), the prevalence rate of male was 34.0% (3 942/11 588), which was higher than that of female 18.0% (1 326/7 377). 2 950 patients (56.0%) completed at least one follow-up of symptom questionnaire and 510 patients (9.7%) completed more than one follow-up of pulmonary function test. Conclusion:The screening and management strategy of COPD based on the IoT and information technology can improve the efficiency of COPD screening, and improve the status quo of under-diagnosis and discontinuous follow-up of COPD.

Objective:To explore a more efficient and standardized scientific research management mode, in order to enhance the best practice of conducting research, improve the agency and satisfaction of scientific research investigators, to improve the efficiency of scientific research management.Methods:According to the problems and deficiencies identified during the process of scientific research management, combined with the latest scientific research management policies and guidelines, the research and development concept of hospital scientific research management platform was formed, and tailored scientific research management information platform was developed by making good use of information technology.Results:After the application of the tailored scientific research management information platform, the efficiency of reimbursement and information access of scientific research personnel was significantly improved, with a significant difference.Conclusions:The construction of scientific research information platform based on the combination of education and management helps to achieve the goal of efficient, standardized and refined scientific research management.

ABSTRACT

Objective:To investigate the characteristics of diagnostic radiology and the situation of repeated examinations at medical institutions of Jinshan district, and put forward strategies and suggestions for the correct guidance on and reasonable applications of various medical exposures.Methods:The cases data on radiological examinations at medical institutions in Jinshan district, obtained in 2017 from Shanghai Municipal Health Commission Information Center Platform, was used to describe the characteristics of population distribution of patients, examination types, examination areas, diagnosed diseases, number of repeated examinations and the result of radiological examinations.Results:The main types of radiological examinations in Jinshan district were the CT scans and the conventional X-ray diagnosis, accounted respectively for 53.2% and 44.7% of the total radiological examinations. The radiological examinations were mainly performed on household registration (77.3%). The number of radiological examinations on males was close to on females, with examination frequency of 49.4% for males and 50.1% for females. The greatest frequency of radiological examinations was found in the age group older than 41 years, accounting for 77.1% of CT scans and 65.2% of the conventional X-ray diagnosis. The main examination sites of the CT scans at Jinshan medical institutions were chest (32.8%), abdomen (19.4%), brain (16.1%) and spine (11.4%), and for the conventional X-ray diagnosis were chest (43.2%) and limbs (39.6%). The main departments applying for radiological examinations are surgery (39.7%) and medicine (27.1%). The numbers of radiological examinations are, respectively, diseases of the respiratory system (19.8%), the injury, poisoning and some other consequences of external causes (15.8%), and the diseases of musculoskeletal system and connective tissue (15.8%). The positive rate of the conventional X-ray diagnosis was 53.1%, and the positive rate of CT scans was 73.5%. The conventional X-ray diagnosis and CT scans contributed higher specific rates of repeated examinations, accounting for 9.3% and 7.9% of the total of such type examination, respectively.Conclusions:CT scans in Jinshan district accounted for more than half of the total frequency of radiological examinations, therefore, interventions should be imposed on certain types of phycians and patients types.

ABSTRACT

Objective:To develop the supportive care information platform based on full cycle needs of patients and explore the application effect of platform in hemiplegia patients with ischemic stroke.Methods:Based on the supportive care scheme for patients with ischemic stroke in the previous study of the research group, combined with literature review and expert group discussion, the supportive care information platform version 1.0 was developed. After 5 patients with ischemic stroke hemiplegia tried for 3 weeks, it was optimized and upgraded to version 2.0 according to the use experience. From June to December 2020, 64 patients with acute ischemic stroke hemiplegia in the Department of Neurology of the First Affiliated Hospital of Soochow University and the Second Affiliated Hospital of Soochow University hospital were randomly divided into intervention group and control group, 32 cases in each group. Barthel Index and balance ability were compared between the two groups. The control group received supportive care plan, and the intervention group received supportive care information platform.Results:After the intervention, the scores of Barthel index in the intervention group were 85 (75,90), 95 (95,100) at 1 and 3 months of illness, which were significantly higher than 65 (55,75), 75 (70,85) in the control group ( P<0.05). After intervention, the scores of balance ability in intervention group were 37 (28,43), 48 (45,51) at 1 and 3 months after illness, which were significantly higher than 28 (16,36), 31(20,37) in control group ( P<0.05). After the intervention, the scores of stroke related knowledge in the intervention group one day before discharge, one month after illness and three months after illness were 14.97 ± 2.98, 19.00 ± 2.14, 22.81 ± 1.60, which were significantly higher than those in the control group (8.69 ± 1.64), (8.88 ± 1.52), (9.09 ± 1.61) ( t values were 109.37-1 169.84, P<0.05). Conclusions:The application of supportive care information platform can provide targeted nrusing measures for patients at different stages of disease development. The platform has good feasibility, and can significantly improve patients′ Barthel index, balance ability and stroke related knowledge, which shows that the platform has good feasihility and clinical application value.

ABSTRACT

Objective To construct and apply a comprehensive information platform for the prevention and control of birth defects based on three-level prevention and control in the birth-age population of Yueyang City, aiming to improve the effectiveness of intervention and reduce the impact of birth defects on the fetus. Methods A retrospective analysis was used to study the married population of childbearing age in Yueyang from January 2017 to December 2019, all of whom were included in the comprehensive information platform for the prevention and control of birth defects based on the three-tier prevention and control. Observed indicators include birth defects I, II and III prevention and control and intervention effectiveness. Results (1) Effectiveness of prevention and control at the first level in each year: After continuous intervention, the education rate, marriage test rate and folic acid distribution in Yueyang city increased year by year, while the incidence of birth defects decreased year by year (P<0.05). (2) Effect of secondary prevention and control in each year: After continuous intervention, the serum screening rate, ultrasound diagnosis rate and amniotic fluid culture rate in Yueyang city increased year by year, while the birth defect rate decreased year by year (P<0.05). (3) Effectiveness of three levels of prevention and control in each year: After continuous intervention, the screening rate of newborns in Yueyang city increased year by year (P<0.05); and the disability rate of CH and PKU was 0% in three years. Conclusions The adoption of a three-level comprehensive prevention and control information platform in the prevention and control of birth defects has the advantages of safe, efficient and stable application, which is conducive to promoting the active conduct of prenatal examinations by pregnant women, enhancing the effectiveness of health education and improving the quality of health care during pregnancy, thereby effectively reducing the incidence, birth and disability rates of children with birth defects and achieving satisfactory graded application results. Accordingly, it is recommended that priority be given to the selection of a three-tier integrated information platform for prevention and control, depending on the actual situation.

The construction process and method of shared TCM pharmacy information platform based on county medical alliance introduced from aspects of overall architecture ,business model and main functions. RESULTS & CONCLUSIONS:The policies for the revitalization and development of medical alliance and TCM ,the needs for the improvement of grass-roots TCM service ability ,and the application of modern information technology provide the feasibility for the construction of shared TCM pharmacy based on county medical alliance. The construction of shared TCM pharmacy based on county medical alliance can broaden TCM list in the primary medical institutions ,integrates and diverts human resources ,save costs ,and provides homogeneous and standardized pharmaceutical care. However ,it is generally affected by non information technology factors such as county regional environment ,policy support ,logistics distribution speed ,cost benefit sharing. T he shared TCM pharmacy information platform based on county medical alliance is mainly based on the TCM alliance led by county-level TCM hospital ,and constructs business model from three aspects :business processing ,business management and business sharing on the basis of the health information platform of TCM department. Its main functions include the whole flow management of TCM decoction pieces , TCM dispensing ,decocting,distribution,pharmaceutical care ,quality control ,convenient service and interface management and so on. The application of shared TCM pharmacy information platform based on county medical alliance helps the opening of the business cooperation and information sharing channel of shared TCM pharmacy based on county medical alliance expands the service scope of TCM ,and enhances the TCM service ability in primary medical institutions.

【Objective】 To facilitate the reimbursement of blood expenses in the hospital through information platform and promote the healthy development of blood donation. 【Methods】 The publicity of blood expense reimbursement was conducted through the hospital information platform,, and direct reimbursement of clinical blood use of voluntary blood donors and their relatives was achieved using the hospital reimbursement management system and Ding Talk office platform. 【Results】 Compared with the traditional mode of reimbursement of blood expenses through blood stations after discharge, direct reimbursement in the hospital simplified the reimbursement process, shortened the distance and time, and improved the satisfaction of blood donors and their relatives significantly. 【Conclusion】 As a new reimbursement model, reimbursement of blood expenses directly in the hospital can improve the satisfaction of blood donors and their relatives, and promote the healthy development of blood donation.

Results of data analysis showed that hybrid training mode integrating online and offline teaching can significantly improve the clinical skill training effect when compared with the traditional teaching mode. Objective@#To construct a national medical radiation protection monitoring information system (NMRPMIS) based on the national radiation health information platform, support the national medical radiation protection monitoring project data reporting, and comprehensively understand the current situation of medical radiation protection.@*Methods@#According to the national radiation protection monitoring plan, laws, regulations and standards, HTML, CSS and JavaScript were used in the front end, JAVA language was used in the back end, SQL server was used in the platform database and Tomcat was used as the middleware. A data reporting and analysis system was set in the system through B/S structure, with statistical analysis of the 2017 annual monitoring data carried out.@*Results@#The business composition of the platform includes four levels: data acquisition (through various business systems), data resource integration (integration of business system public data), application, and display. The national medical radiation protection monitoring system with flexible structure, centralized information and convenient operation was implemented. The national medical radiation protection information was reported in 2017. The basic situation of radiation protection in 33 565 hospitals and the performance and protection information of 10 624 pieces of radiation diagnosis and treatment equipment were reported in 2017. A total of 95 statistical information reports were completed, describing the current situation of medical radiation protection in China from four dimensions: time, administrative geographic information, hospital level, and equipment types.@*Conclusions@#Medical radiation protection monitoring system helped achieve the design goal and successfully complete the data reporting in 2017. It provides data support for understanding the current situation of medical radiation protection and the protection management policymaking in China.

Objective@#To construct a national radiological health information platform, in order to collect and analyze radiological health monitoring and survey data, so as to improve data utilization and ensuring technology support to radiological health.@*Methods@#In accordance with China′s relevant laws, regulations and regulations, a national radiological health information platform was designed, developed and operated in response to the needs of radiological health monitoring, scientific research and management.@*Results@#The system was initially established in 2015, consisting of five business modules: medical radiation protection, personal dose, occupational health, food radioactivity monitoring and occupational radiological disease reporting. In the past 4 years, the system has been running normally and 5 business databases have been established. More than 8 million data of monitoring and survey have been collected. More than 1 400 terminal users have good experiences and frequent interactions.@*Conclusions@#The successful construction of the national radiological health information platform not only promotes and standardizes the network direct reporting of radiological health monitoring and investigation business data, but also provides analytical and utilization tools for technical institutions/personnel on radiological health, and gradually becomes an important scientific basis and database for the development of radiological health laws, regulations and standards in China as well as for national health service.

ABSTRACT

Objective To establish a "patient-centered" integrated information platform for emergency interconnection. Methods Based on the existing software, hardware and network systems of the hospital, design ideas of the modularization, process and standardized were used to reshape the process of emergency diagnosis and treatment in department of emergency-critical care medicine of Huashan North Hospital,Fudan University, and develop integrated information platform for emergency interconnection, including triage, emergency physician workstation, electronic medical records, clinical pathways of key diseases, medical integration, electronic handover classes, imaging, testing, ultrasound, drug counseling and medication safety, performance appraisal and management systems of scientific research, etc. Results The information platform for emergency interconnection was successfully developed. The functional logic of each system was clear and concise. It had strong compatibility, stable performance and powerful processing capability. It could quickly query the target content and support free and fast switching of each window. At present, daily diagnosis and treatment of emergency patients were realized by informationization, which completely liberated the manual labor of medical staff, shortened the processing time of unit patient, and significantly improved efficiency of the work. At the peak of the patients' visit, the overall operation of the emergency department was stable, and all the work was carried out in an orderly manner. There was no delay of the diagnosis and treatment for critically ill patients in the emergency department. Emergency access to patients with key diseases was smooth, and waiting time was significantly shortened. Clinical decision-making of medical staff had been effectively regulated. The success rates of the patients with acute trauma or acute respiratory failure were significantly improved, the time of the critical treatment were significantly shortened in patients with emergency respiratory cardiac arrest or acute ST-segment elevation myocardial, and compliance rate of the door-to-balloon time met the requirements of the Emergency Room to Balloon Expansion Time (DTB) Alliance. Conclusion The integrated information platform for emergency interconnection ran through the whole process of emergency diagnosis and treatment, based on emergency clinical practice, which could meet the needs of daily work in emergency department and help improve the quality of emergency medical and department management.

ABSTRACT

Control of the qualifications and behaviors of medical practitioners in branches of a hospital group has become imperative to guarantee medical quality and safety.We have explored a development program of the information platform to meet the needs of unified authorization and closed-loop management of hospital groups,constructed a medical qualifications authorization management and control platform based on private clouds,established a specialized medical qualifications and authorization model,standardized the approval process,and reduced the approval time.At the same time,we work with a human resources management platform,to develop qualifications assessment tools for supervision,for the purposes of dynamic renewal and withdrawal of medical qualifications.We have set up a centralized medical behavior authority database and an authority verification interface,selected key business steps,and readapted the business system for authority verification.These measures can effectively control practitioners' medical behavior and ensure medical safety.
